Frances M. Schwartz Fellowship Evaluation Form

Evaluation of:  ___________________________________________________________________

To the applicant: Under the Family Education and Privacy Act of 1974, you have the right to examine letters of recommendation written on your behalf after January 1, 1975. Some instructors, however, would prefer to preserve the confidentiality of any statements they make. In order to elicit the most candid evaluations from referees who would prefer to maintain the confidentiality of their letters, you are offered the opportunity to sign the waiver below. Please check one:

(  ) I hereby waive any rights of access I may have to this letter.

(  ) I wish to retain any rights of access I may have to this letter.

The above named applicant has applied for the American Numismatic Society's Frances M. Schwartz Fellowship which is intended to educate qualified students in museum practice and to train them in numismatics, as well as to provide for curatorial assistance in the Greek, Roman, and Byzantine departments. Each year we are able to accept only one applicant; our making the right decisions depends upon your frank evaluation.

It is imperative that evaluations be received in New York promptly.
In the past, otherwise qualified applicants have been turned down because letters of evaluation arrive too late or not at all.
